The home side had only just seen a Kayden Jackson effort ruled out for offside when substitute Simons settled the contest with a composed first-time finish nine minutes from time.
Holding a 1-0 advantage from the first leg, Bolton were the stronger side during a largely forgettable opening 45 minutes before Bradford gained momentum following a triple substitution shortly after the hour.
Jack Bonham produced an excellent save to push Max Power's long-range effort on to the crossbar, while Tyreik Wright's shot deflected narrowly wide. Jackson then believed he had hauled Bradford level in the tie, only for the assistant referee's flag to cut short the celebrations.
The vital moment that sealed the win
Almost immediately, Bolton broke forward down the left and former Chelsea youngster Simons showed his composure with a neat finish from 15 yards.
Bolton will now meet Greater Manchester rivals Stockport County at Wembley on Sunday, 24 May.
