Jair Cunha and Delap had put Forest two goals ahead by half-time, both strikes coming after basic City errors, before Rayan Cherki reduced the deficit with a superb half-volley.
City looked to have seized the momentum, but Forest remained a threat on the counter-attack and Delap produced a spectacular finish to extinguish any hopes of a comeback.
A closer analysis of what happened
In the fog that drifted over the City Ground, Enzo Maresca's side looked completely lost as they slumped to their third consecutive league defeat.
Forest showed courage and aggression from the opening whistle, while City looked fragile and increasingly disjointed.
City have now conceded eight first-half goals in 10 games, with only Crystal Palace (11) and Ipswich (nine) having shipped more in the league this season.
The scrutiny on goalkeeper Gianluigi Donnarumma increases
The former PSG keeper has endured a dip in form in recent weeks, will inevitably intensify.
A strategically placed totem pole might have proved more effective than the Italian for Forest's opening two goals.
On both occasions, the 27-year-old's positioning and reactions left plenty to be desired.
The City keeper was not helped by those in front of him
There was particularly timid defending from Anderson and Gvardiol contributing to Forest's first two goals.
Indeed, only Sunderland (six) and Hull City (five) have made more errors leading to goals in the Premier League this season than City's four.
There was little Donnarumma could have done about the quality of Delap's finish for the third.
Maresca must eradicate such carelessness if City are to keep pace with an increasingly ruthless Arsenal side as the season progresses.
On current evidence, that looks a difficult task.
